We need to know if you, or anyone living with you, has ever been convicted of a serious criminal offence. But you don't need to tell us if the conviction is 'spent'.
| You must tell us about any of these sentences: |

| Prison, youth custody, or young offenders' institution: sentence 6 months or less But you don't need to tell us if the conviction was 7 years ago or more, or three-and-a-half years ago or more if you are under 18. |
| Prison, youth custody, or young offenders' institution: sentence between 6 months and two-and-a-half years But you don't need to tell us if the conviction was ten years ago or more, or five years ago or more if you are under 18. |
| Prison, youth custody, or young offenders' institution: sentence over two-and-a-half years |
| Suspended sentence of 6 months or less But you don't need to tell us if the conviction was 7 years ago or more, or three-and-a-half years ago or more if you are under 18. |
| Suspended sentence between 6 months and two-and-a-half years But you don't need to tell us if the conviction was 10 years ago or more, or 5 years ago or more if you are under 18. |

| Fine, Probation order, Community service order, Combination order, Community punishment order, Community rehabilitation Order
But you don't need to tell us if the conviction was 5 years ago or more, or unless the order has run out, or the conviction was two-and-a-half years ago or more if you are under 18. |
| Supervision order, Care order, Conditional discharge, Binding over But you don't need to tell us if the order has run out, or the conviction was one year ago or more. |
